To register
If you wish to register with the Practice you will need to collect a registration pack from reception.
All new patients will be offered a new patient health check with the Health Care Assistant or the Practice Nurse. Any health issues will be discussed at this appointment and you will be booked into any necessary clinics.
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Change of name, address or telephone number
It is essential that we always have accurate and up to date information for all our patients. If we ever have to cancel a surgery or clinic due to staff illness or any other occurrence beyond our control, we may have to contact you quickly. If any of your personal details change, please let us know immediately so that our records are up to date.
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
